Supported Living Services provide individualized assistance to eligible adults who choose to live in homes of their own. Support can include daily living routines, household tasks, community participation, health and safety, communication, and other areas identified in the individual’s authorized service plan.

Support is tailored to the individual rather than delivered through a one-size-fits-all model. Services are based on assessed needs, personal goals, authorized supports, and the individual’s preferences and routines.

Eligibility depends on the individual’s Regional Center status, assessed needs, Individual Program Plan (IPP), authorized services, and applicable provider requirements.

The process typically begins through the individual’s Regional Center and service coordinator. Approved services and authorizations must be in place before reimbursable services begin.
